–Cancellation Policy–

If you need to cancel an appointment, please let us know 24 hours in advance. If you fail to notify us or does not show for your appointment, you will be charged 100% of the treatment fee. The cancellation fee must be paid before you can schedule another appointment. 

If you feel that you will be more than 10 minutes late, please notify us as soon as possible. 

***The cancellation fee is not covered by your insurance. It will be equal to our cash rate. 

–New Patient Forms–

You may either fill out the new patient forms at our office or fill them out electronically. Please answer all questions to the best of your ability. The more we know about you, the better we can develop a treatment plan for your condition. If you have medical notes and reports of diagnostic images pertaining to your medical condition, please bring them with you. 

***If you want to use health insurance, please relay the information to us before your first appointment. We may not always be able to check your health insurance during your appointment due to the insurance company's hour of operation.
